County Department of Zoning Administration Forms
The links below provide access to forms for various types of zoning actions and permits processed through the Washington County Department of Zoning Administration. A brief description of the use of each form is also provided. Applications for Rezoning of Property and Special Exception Permit require public hearings before the Washington County Planning Commission at the end of the month following the month the application is submitted, and also before the Washington County Board of Supervisors the month after hearing before the Planning Commission (two public hearings are required).

Application for Rezoning (Short Form) - to be completed by property owners seeking rezoning of less than four (4) parcels of property.
Application for Rezoning (Long Form) - to be completed by property owners seeking rezoning of four (4) or more parcels of property.
Application for Special Exception Permit - to be completed by property owners on behalf of themselves or second-party permittees for uses and activities subject to Special Exception Permit pursuant to the County Zoning Ordinance.
Request for Division Sketch Plan/Plat Review - to be submitted along with a sketch plan, preliminary or final plat of a proposed divison of land to be approved by the County for recordation.
Application for Vacation of Plat - to be completed by property owners seeking to legally remove (vacate) specific parcels of record from platted and recorded subdivisions.
